Critically evaluate healthcare technologies for renal dialysis systems.

Objective
You are tasked to provide a case study that details the most important considerations in taking a patient on their journey from the “Advanced Kidney Care Clinic” to being treated on home haemodialysis.
Assignment brief
John is 52 years old and has diabetic kidney disease. He has been under the care of the hospital renal team who regularly monitor his kidney function. He spent several years in the “General nephrology” clinic, before moving to the “Advanced Kidney Care Clinic” which is focused on preparing patients for when their kidney function reduces so much that Renal Replacement Therapy is needed.
John is still working, tries to stay active and enjoys spending time with his children and grandchildren. He has high blood pressure, despite efforts to control it with medication. His biochemistry shows consistently high potassium levels, even though he has received dietetic advice to try and reduce his potassium intake.
John’s kidney Consultant has indicated that his kidney function has dropped to a level where they need to start talking about Renal Replacement Therapy. We would like you to describe the important things that should be considered as John transitions from a patient in the Advanced Kidney Care clinic to being an independent home haemodialysis patient. Things you should include are:
1. Creation of appropriate vascular access
2. What should be discussed when talking to John about the benefits to choosing home HD over in-centre HD
3. What should be discussed when talking to John about the potential issues when choosing home HD over in-centre HD
4. Decisions around the frequency and length of dialysis
5. The dialysis prescription (think about what level of solutes to add to the dialysis fluid)
6. Dialysis equipment required
7. Possible complications with haemodialysis and Safety systems of equipment
8. Commissioning of dialysis equipment
9. Water testing/Good practice
10. Maintaining high quality after installation, equipment, safety, testing, water.
Reminders
– You can assume that the audience who the presentation is aimed at has a good understanding of renal failure and haemodialysis.
– You can get a significant proportion of marks simply for making sure your presentation is easy to read, formatted well, has appropriate references and has a logical flow and structure.
– The rest of the marks will be divided between each of the ten sections outlined above, with higher marks for more comprehensive and relevant information (i.e. things that are particularly important to think about for home haemodialysis rather than just for dialysis in general)
Length:
2000-3000 words. Relevant images, tables or graphs can be included to help illustrate the points you are making. (renal technology)
The Learning Objectives to be assessed in this assignment are as follows:-
1. Knowledge & Understanding
Understand the biology of the kidney and its pathology.
Understand the principles of renal replacement therapy
Critically evaluate healthcare technologies for renal dialysis systems.
Understand the principles of infection control in the dialysis unit
2. Discipline Skills

Demonstrate a detailed knowledge of the underlying pathology of the condition placing the patient at high risk of developing pedal ulceration

LO1: Demonstrate a detailed knowledge of the underlying pathology of the condition placing the patient at high risk of developing pedal ulceration LO2: Describe how the condition impacts on the patient and their feet and recognize how this will affect the management of the patient. LO3: Identify and appraise the research evidence that informs you about the care of the patient. For this essay you need to choose one condition that places a patient at high risk of developing pedal ulceration. The condition needs to be described in detail including an in-depth exploration of the pathological processes that places the foot “at risk”. For example, if you have chosen neuropathy in diabetes, you would not only consider the pathophysiology of the nerves, but also the vascular and immune pathologies that might be related to the changes in the nerves, for example. At L6, we are looking for a wide-ranging understanding of a condition that you might see in a clinical setting. Following on from the pathology of the condition, you need to show a good understanding of the impact the condition on the patient, considering aspects that might impact on your management of the patient. For example, the psychosocial impact of the condition typically needs to be included. Finally, the examiners will be looking for use of evidence to support all areas of your essay, so the next section will be a critical evaluation of a single piece of research that would support your management of the high-risk foot. This might be a critical evaluation of an assessment technique that you would use in the assessment of the foot, or it may be a treatment that could be used such an orthotic / dressing / medication. You need to determine the robustness of the research and reflect on the use of the research in your future practice. You will be required to submit a 2800-word assignment. “You might choose RA and would then be focusing down on the foot deformities and the vasculitis, impact of medication on the foot. ” Can I also take this opportunity to remind you about referencing. I am expecting to see correct referencing in the essay. This is a degree-level piece of writing so format your references correctly as you go along taking the time and effort required to cite them correctly. 1. A brief introduction 250 – 300 You will need to outline the relevance of the topic area. This should NOT be in terms of figures that mean little to the general reader, but should demonstrate an understanding of why this topic is important to those managing the patient. You should define the scope of the essay. (what is going to be discussed in this essay, and why is this important to those managing the patients with RA and pedal ulceration) 2. Main body of first part of essay 1300 You will need to show good academic writing skills to present the work fluently, showing depth of information and ability to link concepts. You need to be explaining clearly the pathology to a level that describes what is happening in tissues and cells. Any molecules / cells you describe need to be clearly explained and you should not assume the reader knows what the molecule / cell normally does. You are expected to reference using the Harvard style throughout the essay. Mention on rheumatoid, whos getting it what what we know about it and why it occurs and the actual DNA understanding of it. Then go on to as far as putting the lower limb at risk then need to think about vasculitis, joint deformities causing overloading which could lead to ulceration and then potentially neuropathy due to nerve entrapment, affect of medication patients are on for rheumatoid that might impact the lower limb ( talk about the medication that have impact on the foot so methotrexate, biologics and steroids) . Then break it down into steps so if focusing on vasculitis (think about what Is happening in blood vessels) this is what the foot is doing and it is putting the foot at risk – think about linking it to the study (there is a lot on early prevention of deformity) if patient ends up with vasculitis and if we help preventing foot from deforming in the first place hopefully the overloading issues will be taken out in the first place to prevent ulceration. Make sure to link the study I am going to critique into the body of essay to make it flow better. When thinking about the psychosocial aspects, if a patient has disability of their hands how will they put on their footwear and wear orthotics, think about the depression will they listen to instructions, take medication listen to advice, could be that they have to wear certain footwear for the orthotics that to fit would be ugly and then they wouldn’t want to wear them. 3. A critical exploration of the evidence. 800-1000 Identify one piece of research that is relevant and recent. If you chose an old piece of research, it needs to be a seminal piece of work. You should write a critique of this paper Control trial and not randomised, look for a quality of research paper checklist, for after study where the person is their own control, what is good about Study to critique: “The effect of pre-fabricated insole on plantar pressure distribution in patients with rheumatoid arthritis” 4. A coherent, reflective conclusion 250 – 300 Your essay should finish with a summary of the key information given and a reflection about how the knowledge gained may impact on your management in your future practice and linking the research paper and the theoretical ideas from the first section.

Outline at least three potential risks from the quality improvement and patient safety initiative.

Task For this assessment you will be given information relating to the proposed introduction of a quality of care initiative in a health service. You are responsible for writing a brief regarding the potential risks and benefits of this initiative to the health service board and to make your recommendation about whether or not to adopt the initiative. What is a risk analysis? A risk analysis brief is a short formal written communication that highlights the key risks in relation to a particular proposal. In this case, the brief will address the risks of a quality improvement and patient safety initiative for which you will be provided details. While any risk appraisal is grounded in objective assessment, you must still apply a certain degree of persuasive language to your writing to encourage people to read or listen to what you are telling them. You need to think about the audience reading the brief, and how you can be convincing without straying from the factual. I have provided a template for you to work from, showing the key components you should include. Assessment content The brief will include the following key aspects: Section 1: Provide a short introduction to the quality and safety issue that has led to this quality and safety initiative, with reference to how this addresses continuous quality improvement (i.e. was the change stimulated by a need, a negative event, cost blow out, and so on). Section 2: Complete a risk analysis table which covers the following elements: – Outline at least three potential risks from the quality improvement and patient safety initiative. – Summarise the relative severity and likelihood of the associated risks. – Summarise how potential risks might be minimised. – Provide an indication of your assessment of the relative importance of the risk in relation to the potential benefits. References In-text referencing and references lists are not included in the word count. A list of references is to be included at the end of the short answer section. Formatting and style of references should be APA. 15 references to be used. Word count: 700 words Please complete the assignment using the template provided. I will message you with a sample assignment completed by a friend which can be used as a guide. Please ensure to keep the content original (not plagiarised) but feel free to use ideas from it and references as needed.

For this assignment you will submit one paper with two parts

For this assignment you will submit one paper with two parts
Part D: Change Model
Roger’s diffusion of innovation theory is a particularly good theoretical framework to apply to an EBP Project. However, learners may also choose to use change models, such as Duck’s change curve model or the transtheoretical model of behavioral change. Other conceptual models presented, such as an utilization model (Stetler’s model) and EBP models (the Iowa model and ARCC model) can also be used as a framework for applying your evidence-based intervention in a health care setting.
In 300 words, apply a change model to your implementation plan.
• Apply a change model and carry your implementation through each of the stages, phases, or steps identified in the chosen model.
• Create a conceptual model of the project. The conceptual model should be placed in the appendix.
Part E: Implementation Plan
In another 600 words, provide a description of the methods to be used to implement the proposed solution. Include the following:
• Describe the resources (human, fiscal, and other) or changes needed in the implementation of the solution. Consider the clinical tools or process changes that would need to take place. Provide a resource list. The resource list should be placed in the appendices.
• Use strategical analysis to establish the feasibility of the implementation plan. Address the costs for personnel, consumable supplies, equipment (if not provided by the institute), computer-related costs (librarian consultation, database access, etc.), and other costs (travel, presentation development). Make sure to provide a brief rationale for each. Develop a budget plan. The budget plan should be placed in the appendices.
• Describe any specific barriers (real or potential) that will need to be assessed and eliminated. How will you manage barriers that cannot be eliminated?
• Describe the amount of time needed to complete this project. Create a timeline. Make sure the timeline is general enough that it can be implemented at any date. Summarize the timeline in your narrative and include a graphic of your timeline in the appendices.
